The Senior Care Coordinator is a health and human services specialist who guides families and Bethesda staff as they care for their family member/resident. The Senior Care Coordinator is a trained and experienced professional who combines his or her working knowledge of health and psychology, human development, family dynamics, public and private resources and funding sources with a specialized focus on issues related to aging and elder care while advocating for the individual throughout the continuum of care. The Senior Care Coordinator assists older adults and persons with disabilities in attaining their maximum functional potential and independence. In addition, the Senior Care Coordinator is an experienced guide and resource for families of older adults and others with chronic needs, including helping those suffering from Alzheimer’s Disease or Parkinsons or exhibiting symptoms of dementia. The Senior Care Coordinatorr is familiar with community resources that continue to support the independence of seniors in his/her service area. The Senior Care Coordinator is an integral part of connecting and maintaining residents and their families with all levels of the continuum of care within Bethesda.

Job Qualifications

  • Education/Training/Experience
    • LCSW Degree
    • Two – five plus years of recent progressive experience in health and/or related field.
  • Licensing/Registration/Certification
    • LCSW required
  • Geriatric Care Management certification preferred
  • Current driver’s license
  • Skills and Abilities
    • Knowledge of regulatory requirements and management processes.
    • Excellent oral and written communication skills.
  • Computer skills.
